Your child's safety and health are our priority. Please help us by adhering to the following:
- Please provide the school nurse with a copy of the Universal Health Certificate, Dental Certificate, and updated immunization records. These documents are required every school year.
- All medications must be brought to school nurse by the parent/guardian accompanied by the appropriate form signed by the physician (Asthma action plan, Medication Authorization, etc.). No medication will be accepted from the student.
- Please update your telephone numbers with the school registrar for emergency purposes.
- Please inform the school nurse if the student has an injury that requires a cast, splint, surgery or special accommodations.
Please inform the school nurse when your child has been absent for more than 3 days for an illness.
